2000 Ikelite Snapshot Photo Contest

(Print this form and attach to your entry, or obtain a printed form from your Ikelite Dealer)

Instructions:

Entry forms must be filled out completely to qualify as an official entry. Please follow these directions:
  1. Attach a label to the back of each Snapshot entry with title of photo, your name, address and phone number.
    Do not mark on back of photo.
  2. Include a photo of yourself (optional) along with a short, personal diving "resume."
  3. On a separate piece of paper write a few brief but interesting statements describing the dive you were enjoying when you took this contest photo(s).
  1. Include the name and address of your local Ikelite Dealer.
  2. Submit by November 30, 2000 (postmarks after this date will not be accepted).
  3. Send Entries to:

I understand that by entering the Ikelite Snapshot Photo Contest my submitted photo(s) along with my "article" will appear on the Ikelite Web site if my photo(s) is selected as one of the finalists. I also understand that Ikelite may use my photo(s) for promotional concerns even if it is not selected in the final cut. Furthermore, I confirm that I am not a professional photographer, that this is my photo, that it was taken with an Ikelite Aquashot or Auto-35 System, and is not the work of another individual. Remember to indicate the type of camera used.
